New Student Information

Welcome to Saint Mary’s

Student Health Services seeks to foster an environment that encourages healthy living and supports you in times of illness. In order to do this, we need your help! There are specific requirements for all new and transfer students. In addition, there are recommended actions that we encourage each of you to take before coming to college.

Please complete the following prior to your first semester at Saint Mary’s University (find full descriptions below the checklist):

REQUIRED
◻ Student Health History Form (recommend submitting after a health exam)
◻ Immunization History Form (recommend submitting after a health exam)
◻ Tuberculosis (TB) Screening Questionnaire
◻ Guardian Medical Consent for Minors Form (if younger than age 18 at the start of the semester)

Failure to complete required documentation will result in a health hold and prevent you from registering for classes for the next semester.

RECOMMENDED
◻ Schedule a pre-college health exam
◻ Receive all recommended vaccinations and document them in the Student Health Portal
◻ Contact insurance to verify local health insurance coverage while at Saint Mary’s
◻ Obtain a copy of your health insurance card for personal use

Student-athletes: Any forms submitted to the Athletics Department do not replace or satisfy the requirements for Health Services. Health documentation must be submitted through the Student Health Portal.

Student Health Portal

Your Student Health portal provides you with online access to appointment scheduling, health records, confidential messaging with healthcare providers, and online form completion. Log in using the same username and password you use to log into your Saint Mary’s email account. All health forms will become available in your health portal in June and should be submitted electronically.

Pre-college health exam:

A general physical is recommended for all students prior to entering college. In addition to reviewing your health status, this provides an opportunity to complete recommended vaccinations. If you have health records you would like to share with health services, you can upload through the Student Health Portal. We recommend you wait to complete your immunization and health history forms until after your general physical in order to provide the most up-to-date information. Once forms have been submitted, you will not be able to modify them.

Health History Form:

You’ll need to complete a Health History Form in your Student Health Portal. This provides basic health information such as health conditions, medications, and allergies and allows health services staff to more effectively support you.

Immunization History Form:

Minnesota state law requires that we collect documentation of measles/mumps/rubella (MMR) and tetanus/diphtheria (Td or Tdap) vaccination. Two doses of MMR are required after the age of 12 months. Tetanus/diphtheria vaccination must be within the past 10 years. Additional vaccinations (Hepatitis A and B, Varicella, HPV, Meningococcal ACWY, Meningococcal B and COVID-19) are strongly recommended prior to beginning college. Tuberculosis (TB) Screening Questionnaire:

All students should complete this brief questionnaire to determine whether additional tuberculosis screening is required.

Guardian Medical Consent for Minors Form:

Parents or guardians of students who will be younger than age 18 at the start of the semester should print, sign, and upload this form to authorize Saint Mary’s healthcare staff to provide care for minor students. This form can be printed from your Student Health Portal.

Insurance:

We strongly recommend that all students have health insurance. We recommend that you contact your insurance provider to check coverage, should you require services off-campus during your time at Saint Mary’s. We encourage students to obtain and carry a copy of their health insurance card to facilitate filling prescriptions or receiving off-campus services, if needed.

This includes assessment and management of acute illness and support of student health for ongoing conditions. Health education, health promotion and mentorship provide long-term benefits to students as they learn to navigate today’s complex healthcare environment and appropriately utilize limited healthcare resources.

Confidentiality:

All Wellness Center appointments and records are held in strict confidence. Information will not be shared with parents, faculty, administration, peers or anyone else without the student’s consent. An exception is made if a life-threatening situation arises.

Schedule an Appointment:

You will need to set up an appointment through the My Winona Health portal. If you already have a portal account, you can skip to the attachment which walks you through how to make the appointment. If you don't have a portal account, follow the instructions below.

If you don't have a portal account,you must call 507.457.7648 to register yourself in the Winona Health system before you are able to make an appointment. Be sure to mention that you are a Saint Mary's University student. They will ask for some basic information such as name, birthdate, address, and Social Security Number. Your SSN is to serve as a security question. If you don't have it, a different security question can be used. Once this is completed, you will receive an email with a direct link to make an account for the MyWH portal. This link is only active for 24 hours. You should receive this link immediately. If you don't, call back and ensure they heard your email address correctly. You can then schedule on the My Winona Health portal or through their app going forward. Instructions for how to navigate the portal once you reach that step are below.

All services on campus are free for all students, however if you choose to see a provider off campus you will be responsible for all incurred costs.
